• Join in developing and maintaining a comprehensive health record, by authorizing delivery and flow of clinical information to and from clinicians and health care facilities.

• Provide Exclusive (VIP/MD) Health Management with feedback, regarding the agreed upon treatment plan for the patient.

• Communicate Openly and Honestly about your health. Must be fully cooperative and proactive with Exclusive (VIP/MD) Health Management’s staff, our exclusive physician and their staff.

• Attend all follow-up appointments made by Exclusive (VIP/MD) Health Management, our exclusive physicians, and all scheduled appointments by the patient.

• Must be an active participant in the development stages with the physicians “Call to Action” plan and treatment for patient’s chronic or acute condition. Once all parties have agreed to the treatment plan, the patient must follow through on the agreed upon treatments and follow the plan accordingly.

• Patient must submit all requested materials, samples, and any additional information as it is requested by Exclusive (VIP/MD) Health Management or the exclusive physician.

• Once all parties agree upon a Plan of Care, participate in the exclusive physician's recommendations that has to do with the patient’s care or improvement of the patients’ health and wellness.

• Appear on time for appointments, procedures and for all medical tests at the exclusive physician’s office, Exclusive (VIP/MD) Health Management’s office and at any other facility.

• Participate in all action plans and goal setting with respect to maintenance or improvement of the patient’s health and well-being.
